Here is a link to the FINA swimming rules. Items SW4 to SW9 discuss the rules for each stroke. Also below is a summary of some common disqualifications, but please refer to the FINA guide for more detail.
Some common disqualifications
Butterfly

touching at the finish or turn with only one hand

legs fluttering slightly (like in front crawl) rather than dolphin kick - feet must stay together

arms not brought sufficiently out of the water
Breaststroke

touching at the finish or turn with only one hand

not doing proper 'frog legs' kick - should be level with feet facing outwards

elbows out of the water

arms going past the waist during the stroke
Backstroke

turning onto front when touching the wall to finish
